Every relationship with AgingCareAlly begins with a complimentary 15-minute introductory consultation. This confidential conversation allows us to understand your situation, determine whether our services are the right fit, and identify the most appropriate level of support moving forward.

Comprehensive Assessment

Targeted Care Consultation

A focused, streamlined assessment designed for specific concerns or transitional decisions. Ideal for families seeking expert guidance on a defined issue. Services are billed hourly based on scope.

Ongoing Advisory & Coordination

Structured hourly engagement providing continued support through implementation, monitoring, and coordination among healthcare providers, community resources, and family members.

Care Coordination Engagement Package

Available for clients purchasing 12 or more hours of advisory services, this package is designed for periods requiring intensified coordination and proactive oversight during complex or evolving transitions.

Comprehensive Care Assessment & Strategic Planning

Every engagement begins with clarity.

A structured evaluation of medical status, cognitive function, safety risks, living environment, support systems, and personal goals establishes a foundation for informed decision-making.

Care Coordination & Advocacy

Coordinated oversight across physicians, hospitals, rehabilitation facilities, specialists, insurers, home care providers, and community resources. Includes appointment coordination, provider communication, local representation, regular updates, and logistics management.

Crisis Support & Complex Care Navigation

Support during hospitalizations, rehabilitation stays, sudden diagnoses, cognitive decline, and changes in living environment.

Includes discharge coordination, rehab and facility liaison oversight, crisis response, and stabilization planning.

Aging Care Consulting & Planning

Continued monitoring for individuals at home or in residential settings.

Includes home safety planning, vendor coordination, medication oversight, appointment management, and proactive check-ins.

Dementia & Cognitive Support

Specialized guidance from a Certified Dementia Practitioner (CDP).

Includes safety and behavior plans, family education, environmental adjustments, and evolving care strategies.

Solo Aging Support

Advance directive facilitation, future-care roadmaps, legal and surrogate coordination, and structured preparation for individuals and solo agers seeking proactive planning.

Family Communication & Care Partner Support

Support for spouse caregivers, adult children, conservators, and out-of-area families.

Includes local oversight, regular reporting, caregiver coaching, respite planning, and family mediation.
