WHAT WE DO

Care Coordination & Advocacy

Guidance, communication support, and coordinated care across providers.

Dementia & Cognitive Support

Specialized strategies, education, and navigation for cognitive decline.

Crisis & Transition Support

Immediate help during hospitalizations, unsafe situations, or sudden changes.

Care decisions during aging are rarely simple. Systems are complex. Information is fragmented. And too often, individuals and families are expected to navigate alone.

AgingCareAlly was created to bring structure where there is uncertainty, access where there are barriers, and steady advocacy where every voice deserves to lead.

About Erika Austin, CMC, LPN, MBA‑HCM, CDP, Aging Life Care Professional®

My passion for geriatric care began long before my professional career. I watched my own family care for loved ones without resources, support, or healthcare experience. Seeing them struggle to navigate complex medical decisions ignited my commitment to helping families avoid that same overwhelm.

My early work as a CNA deepened that commitment. I witnessed seniors’ voices go unheard — especially those living with dementia — and it fueled my determination to become a strong advocate for older adults and their families.

As a Licensed Practical Nurse with extensive bedside experience, I saw firsthand the gaps in communication, coordination, and follow‑through that often leave seniors and families confused, unsupported, or unsafe. These experiences shaped my belief that every older adult deserves a knowledgeable advocate who understands the system and ensures their needs are not overlooked.

Today, as a NACCM Certified Care Manager (CMC) with 30 years of clinical, operational, and healthcare leadership experience, I bring both personal understanding and professional expertise to every client I serve. As an Aging Life Care Professional®, Certified Dementia Practitioner, and healthcare strategy consultant, I help families, care partners, and solo agers navigate medical complexity, dementia care, hospital transitions, and long‑term planning with clarity, compassion, and steady guidance.

I founded AgingCare Ally to be the advocate I wish my own family had — a trusted partner who listens deeply, understands the system, and walks beside you through every step of the aging journey.

Why This Work

The foundation of AgingCareAlly began long before professional titles. As a child, our founder watched her family care for aging relatives during serious illness.

They acted with devotion and strength, yet without clear guidance about available resources or how to navigate complex medical systems.

She witnessed firsthand how complex and overwhelming critical decisions can become without a knowledgeable partner. That early understanding shaped a professional path into healthcare — and ultimately the formation of AgingCareAlly.

At AgingCareAlly, the foundation of our work is built upon decades of experience in direct patient care and healthcare transitions. Starting with bedside advocacy while working as Certified Nursing Assistants with individuals living with dementia, we learned to notice subtle changes, protect dignity, and speak up when something wasn’t right. As Licensed Practical Nurses and clinical leaders, we developed individualized care plans, coordinated multidisciplinary teams, and ensured patients received thoughtful, aligned care that reflected their unique goals.

Through years of collaboration with hospitals, rehabilitation facilities, case managers, insurers, pharmacies, and community providers, we identified critical areas where systems break down, communication falters, and individuals are left navigating complex medical situations without adequate support.

AgingCareAlly was created to fill these gaps, providing structured care coordination, guidance, and advocacy to ensure families and individuals can confidently navigate the challenges of aging with clarity, dignity, and alignment with their goals.

The work of AgingCareAlly matters because periods of transition—whether medical, cognitive, or situational—require both clarity and steadiness. Whether we’re supporting an adult child navigating care decisions, a spouse walking beside a partner through illness, or a solo ager planning proactively, our structured guidance and thoughtful oversight ensure that what matters most is protected.

For families, AgingCareAlly provides disciplined care coordination and clinical advocacy. We anticipate barriers, organize next steps, and ensure that care reflects both medical best practices and personal values, giving families the confidence that their loved ones are in the right hands.

For care partners, our structured guidance and clinical oversight reduce fragmentation and bring clarity during times of significant change. Decisions are not made alone; they are informed, thoughtfully aligned, and supported by expert advice.

For solo agers, our engagement begins with understanding individual goals, preferences, and definitions of quality of life. All recommendations are driven by these priorities, ensuring that independence is preserved and future planning remains intentional, with the right support at every stage.

Committed to Your Loved One's Wellbeing

Our guidance and management ensure safe, organized, and compassionate care during every transition — helping families navigate aging with confidence.
